TRAINING PORGAM FOR  3rd year NURSING STUDENTS, BACHELOR OF SCIENCE IN NURSING, NATIONAL UNIVERSITY, MANILA Year 2023

RELATED LEARNING EXPERIENCE: HEMODIALYSIS UNIT

  1. INTRODUCTION 

Nursing education involves practical training provided to students with the purpose to prepare them for their future duties as nursing care professionals, and in order to apply skills into practice, clinical experience is important to the students.

Clinical placement is mandatory in the training of student nurses and for applying theory into practice, leading to the development of competency in the nursing profession.

The clinical placement learning experience provides students the opportunity to interact with patients and community members, ensuring that clinical skills are obtained, underpinned by a sound knowledge base for them to become competent and independent practitioner. 

This clinical education is provided to Nursing students by experienced college teachers, Clinical instructors nursing preceptors who are qualified in this educational task and utilizes techniques to organize and help monitor the progress of the students’ clinical experience and supervision.

  1. OVERVIEW 

According to the American Nurses Association, Nephrology has been a recognized specialty for over forty- five years. Over the years, patients with End Stage Renal Disease have dramatically increased which necessitated the availability of kidney disease treatments thru renal replacement therapy.  This resulted in the increased need for nephrology or dialysis nurses in many health care settings.

Hemodialysis is a procedure where a dialysis machine and a special filter called an artificial kidney, or a “dialyzer”, are used to clean the blood when the kidneys are no longer healthy enough to do their work adequately. To get your blood into the dialyzer, the doctor needs to make an access, or entrance, into your blood vessels. This is done with minor surgery, usually in the arm.

Hemodialysis is a four-hour treatment procedure and only Certified Hemodialysis Nurses are allowed to perform such. Licensed nurses are trained and undergo comprehensive learning experience to develop them to be expert and competent in the field of hemodialysis nursing.

3. BACKGROUND:  KIDNEY FOUNDATION OF THE PHILIPPINES INC. RENAL CENTER

KIDNEY FOUNDATION OF THE PHIL. INC. RENAL CENTER is a non-stock, non- profit organization that meet the challenging challenges in saving lives and extending quality life of patients with kidney disease.

It has modern dialysis facilities, assuring quality dialysis treatment manned by qualified and duly-trained doctors, nurses and dialysis technicians. 

In 2017, KFP launched the “Advance Hemodialysis Training Program for Nurses” which  was accredited as a CPD Provider by the Professional Regulation Commission. Up to present, this training program is continuously conducted and offered to Licensed nurses to be certified as a hemodialysis nurse.

NATIONAL UNIVERSITY -MANILA, an educational institution duly recognized by the Commission on Higher Education which offers Bachelor of Science in Nursing to its students where Related Learning Experience (RLE) is required as part of their course curriculum. 

This institution wishes to affiliate with Kidney Foundation of the Phil. Inc. in order to provide its nursing student with clinical experiences, related to their activities that will help accomplish its objectives in giving its students the best possible education available. 

4. OBJECTIVES OF THE TRAINING PROGRAM 

4.1 Provide nursing students with clinical experience (“know and how”) by actual nursing activities in the hemodialysis unit which includes:

  • Checking the vital signs and talking to the patient to assess their condition prior to start of the hemodialysis treatment.
  • Explaining the hemodialysis procedure.
  • Educating the patient about treatment process.
  • Overseeing the dialysis treatment from start to finish. 
  • In some instances, setting up the hemodialysis equipment, ensuring all supplies needed is available, preparing patient and start administration of the treatment.

4.2 Educating the students in preparing the patient for dialysis by:

  • Assessing the patient whether the blood pressure indicates hypertension or hypotension
  • Assessing the patient’s temperature for fever, which could indicate infection.
  • Assessing rate and quality of respiration 
  • Assessing patency of the vascular access (Arterio-venous fistula or temporary catheter)
  • Avoiding taking of blood pressure in the patients arm with Arterio-venous fistula or Arterio-venous graft to avoid damage of the vascular access.

4.3 Educating the student on the responsibilities of the nurse during dialysis by:

  • Obtaining vital signs periodically every 30 mins
  • Observing the patient’ proper body alignment, allow frequent position changes with assistance. 
  • Monitoring for episodes of nausea and vomiting, hypotension, cramps, chills etc. which may occur during dialysis
  • Managing complications that arises during dialysis.

4.4 Educating the student about care of the patient after dialysis by:

  • Assessing the vascular access for any bleeding or hemorrhage.
  • Avoiding trauma to the arm with vascular access when moving or assisting the patient in ambulating.
  • Assessing the vascular access for ballooning or bulging that may indicate aneurysm that can rupture and cause hemorrhage. 

4.5 Educating the students on proper documentation in the patient’s medical file by:

  • Recording accurately patient’s information, treatment plan, procedure and outcomes. 
  • Being precise when recording information, administering treatment, ensuring patient’s safety and tracking their progress. 

5. METHOD OF INSTRUCTION

  • Classroom discussion 
  • Actual clinical exposure in the dialysis area under supervision of the KFP Preceptor. 

6. AFFILIATION DATE / DURATION 

  • October 2, 2023 onwards as stated in the MOA

7. PARTICIPANTS: (240 Nursing students) 

  • 3rd year Nursing students – 10 students per 2 weeks, 8 hours per day of duty, total of 48hrs/group
  • 6am to 2 pm / 8am to 4pm (duty schedule) 

8. REQUIREMENTS: to be provided by National University)

  • Hepatitis profile (HBsag. Anti HCV, Anti HBs). Record of vaccine if there is any.
  • Chest X Ray – 6 months
  • If with symptoms of fever and cough, Antigen swab is done at KFP 

9.  UNIFORM (to be provided by National University)

  • Scrub suit with smock gown
  • School Identification Card

10. PRECEPTORS 

KFP preceptor for every 4 to 5 nursing students 

11. EVALUATION – NATIONAL UNIVERSITY REQUIRMENT

CLINICAL PERFORMANCE shall be based on the following criteria:

Performance Evaluation (Clinical area) 40%

Pre-test / Post test 10%

Learning Feedback Journal 5%

Attendance/Attitude 10%

Evidence Based Practice 35%

Note: Evaluation of the Nursing student shall be submitted to National University Nursing Coordinator within a week after the completion of the RLE.
